TIPS FOR USING THIS WEBPAGE SUCCESSFULLY

When you click on a link a new window (tab) will open in your browser.  You must press your browser’s back button to return to the main webpage.

Do NOT press the [Enter] key when you are filling out a form.  This can result in the form being submitted prematurely.

You can only complete one Work Search Activity Log at a time. If you need to submit multiple (weeks) forms, then you must exit out of the webpage entirely, then access the main webpage again to start a new form. The Refresh button is NOT an option.

As a participant in the Reemployment Services and Eligibility Assessment (RESEA) Program, you are required to attend an Initial RESEA review and a follow up RESEA Review Meeting.

IMPORTANT!  Forms MUST BE submitted no later than one hour prior to your scheduled appointment.

PLEASE NOTE:  Make a note of your JobQuest Job Seeker ID. This is also your MassHire Job Seeker ID Number .- you will need this to complete your RESEA forms!

FOR THE INITIAL RESEA REVIEW APPOINTMENT, YOU NEED TO:

  1. Complete an Unemployment Insurance Eligibility Questionnaire.
  2. Submit Work Search Activity Logs for each week you have claimed up to the time of your Initial RESEA Review appointment.
    • If you have submitted your work searches in your UI Online account then you can access them by following the directions in this link  HowtoObtainWorkSearchLogSubmittedviaUIOnline and e-mail them to your counselor or use the document upload at the bottom of this website.
    • If you need to complete your logs, scroll down to the Work Search Activity Log section on this webpage (you will need to do a separate form for each week you are claiming)
  3. Submit a resume. If you already have a resume, you can either;
    • upload it into your JobQuest account.
    • e-mail it to your counselor.
    • use the document upload at the bottom of this website.

If you do not have a resume, you can complete the Resume Data Sheet below, or create one using the MassHire Central Pro Resume Maker then upload or email it to your counselor.

FOR THE RESEA REVIEW APPOINTMENT, YOU NEED TO:

  1. Complete an Unemployment Insurance Eligibility Questionnaire – YES, you need to do this again!! See #1 above.
  2. Submit Work Search Activity Logs for each week you have claimed from your Initial RESEA Review meeting up to your RESEA Review appointment. See #2 above.
  3. Submit a resume – Skip if you completed it at your Initial RESEA Review meeting.
  4. Complete Labor Market Research Worksheet.

Purpose:  Fill out this work search log to record your job search activities, to use in your MassHire Career Center appointments, and to verify you are looking for work.  This log covers three work searches for ONE week.

You must complete a log for each week you claim benefits.   FAILURE TO SUBMIT A COMPLETE FORM MAY RESULT IN DISQUALIFICATION FROM RECEIVING BENEFITS AND A POTENTIAL OVERPAYMENT FOR BENEFITS ALREADY RECEIVED.
